Abstract

Type 2 diabetes mellitus (T2DM) is a chronic disease that needs long-term blood glucose management to help reduce the risk of complications. Empagliflozin and insulin glargine are commonly used treatments for T2DM, but they have different mechanisms of action, clinical benefits, and adverse effects. This review compares the mechanisms of action, clinical evidence, safety profiles, and economic considerations of the two medications. Empagliflozin is a sodium-glucose cotransporter 2 (SGLT2) inhibitor that lowers blood glucose by increasing urinary glucose excretion without depending on insulin. It can also contribute to weight reduction and has a lower risk of hypoglycemia when used alone. In comparison, insulin glargine is a long-acting basal insulin analogue that provides continuous insulin activity, helping improve fasting blood glucose and overall glycaemic control. In an observational study of patients with inadequately controlled T2DM, empagliflozin-based therapy was associated with greater reductions in HbA1c, fasting plasma glucose, and body weight compared with insulin glargine-based therapy. The study also reported fewer hypoglycemia events with empagliflozin-based therapy. However, these findings do not necessarily indicate that one treatment is generally superior to the other, as both medications have different benefits and potential risks. Empagliflozin may increase the risk of genitourinary infections, while insulin glargine is more commonly associated with hypoglycemia, weight gain, and injection-related adverse effects. Economic factors may also influence treatment decisions, because medication costs and other healthcare expenses can vary between the two treatments. Overall, both medications can be effective treatment options for T2DM, and the choice between them should consider the patient's clinical condition, treatment goals, risk of adverse effects, and economic factors.
